  • Form 6: Application for registration of new electors turning 18 on any of the four qualifying dates, including advance applications filed during the annual draft publication period.
  • Form 7: Application for objection to an entry or deletion of a name due to death, permanent shifting, or duplicate registration.
  • Form 8: Application for shifting of residence (within or outside the assembly constituency), correction of existing entries in the electoral roll, replacement of Elector Photo Identity Card (EPIC), or marking of Persons with Benchmark Disabilities (PwD).
  • Form 6A: Registration of overseas Indian electors holding valid Indian passports under Section 20A of the Representation of the People Act, 1950.

Booth Level Verification and Polling Station Rationalization

Before publishing the draft electoral roll for an Assembly Constituency, Chief Electoral Officers (CEOs) and District Election Officers (DEOs) conduct physical verification of polling stations. According to Election Commission of India manuals, polling stations are rationalized to ensure that voters do not ordinarily travel more than two kilometres and that each booth serves a manageable elector threshold with assured minimum facilities (AMF), including ramps, drinking water, adequate lighting, and accessible signage.

Booth Level Officers conduct house-to-house verification alongside recognized political parties' Booth Level Agents (BLAs) to identify uncollected EPIC cards, multiple entries, and deceased voters. No deletion is carried out suo-motu during an election cycle without issuing statutory notice and recording an speaking order by the Electoral Registration Officer.

Citizens across India can verify their assembly constituency, part number, serial number, and polling station address through the official Voters' Service Portal (voters.eci.gov.in) or the Voter Helpline mobile application using their EPIC number or registered mobile details.
